Use this form to authorize a charge in your credit card for your $200 deposit or your booking full amount.

2. complete it
3. send it by fax to Academia Tica at +506-292-7136
4. include: copy  of your credit card (both sides) and a copy of the main pages of your passport (include picture and signature pages)Without them, the credit card company will not process this authorization and your booking will not be completed. VISA is the most accepted credit card in Costa Rica.
